If a young person has an EHCP on admission, we will communicate closely with the home school in order to monitor targets through assess, plan, do, review (APDR) and collaborate to set new targets within annual reviews.
If we feel that a young person would benefit from an EHCP following discharge, we will support the parents/carers and home school through the application process.
